Username: First Letter of Your First Name, First 4 Letters of Your Last Name, First 4 Digits of NPN License Number
Password: First 4 Digits of NPN License Number

Username: First Initial of First Name, First 3 Letters of Last Name, Last 4 of SSN
Password: Date of Birth (mmddyyyy)
Example: John Young (SSN: 000-00-1234) (DOB: 01/30/1950)

Password: 01011950
