*The Cleveland Hill UFSD is requiring for those individuals electing to take health care through the district that you answer each portion of this enrollment registration and attach a copy of the following for your spouse or children over 1 year old - social security card, birth certificate, and marriage license if you are newly married.

*The Cleveland Hill UFSD is requiring for those individuals electing NOT to take health care through the district that you answer each portion of this enrollment registration and attach a copy of the following - the front and back of your health card if insured under another group plan. You must also complete the Waiver of Coverage and Letter of Attestation forms.

*The Cleveland Hill UFSD is requiring for those individuals looking to waive the HRA that you complete the HRA Opt-Out Form. This should only be completed by those who are 1. Not taking insurance through CHUFSD AND 2. Are on the Health Exchange OR Not able to participate in the district's HRA.
