The annual Form 1095C is now available for calendar year 2025. This form is essentially proof that an affordable offer of coverage has been made to an employee and that he/she and their family has health insurance. Employee can obtain a copy of their Form 1095C by logging into this website or callng Ethan Allen Benefits Service Center at 888-909-6854.

Username: ETH combined with your six digit clock number (e.g. clock number is 001234, you login would be = ETH001234). Be sure to include all six digits including zeroes.

Password: All Passwords have been reset to the last four digits of your Social Security Number as of 9/25/2026.
