To access the site for the first time, please use the following:

Employees/Employers
 Username: First Initial of First Name, First 3 Letters of Last Name, Last 4 of SSN
Example: John Smith (SSN: 000-00-1234) = JSMI1234

Password: Date of Birth
Example: John Smith (DOB 1/23/1991) = 01231991

Brokers
 Username: First Initial of Your First Name, First 3 Letters of Your Last Name, Last 4 Digits of Primary Broker's License Number
Example: John Smith L1234567 = JSMI4567

Password: 4 Digit HealthPass Broker ID of Primary Licensee

You will be required to change your password after your initial login.
