Username: First Name + First Initial of Last Name + First Five Digits of SSN + Birth Year (ex. JamesS123451977)

Initial/Default Password: Lower case first initial of first name + 2 digit birth month + first five digits of SSN

You will be asked to change your password after your initial login. Your username is not case sensitive. Please use your legal name for both username and initial password.

If you have forgotten your password or are having trouble logging in, please click on the "Forgot Password?" link to reset it.
