The benefits website is a resource for all of your benefits needs and will give you access to:

- Enroll in, change, or view current benefit elections

- Update your beneficiary designation

- View plan design summaries

- Claim forms

- And much more!

You can access the benefits website 24 hours a day, 7 days a week.


Login Instructions:

Enter your Username and Password.

Username: Your Username is your Employee ID Number (EID) which is located on your paystub. Do not include the preceding zeros when entering your EID.
(Example: If your EID on your paystub is 07458, please enter 7458 as your username.)

Password:
First Time User: Your password is the last 4 (four) digits of your social security number.
Returning User: Use your previously created password or the "forgot password" option.

Please note in the subject line "PASSWORD REQUEST" and include your first and last name, and company name- Destination Maternity. Do NOT include your social security number. You can include your EID.

Please note that your password will be reset to the last 4 (four) digits of your social security number. You will receive an email confirmation from the Benefits Help Desk once your request is processed.
