Username - Your first name followed by first initial of last name and last 4 digits of your SSN.
(Example: John Smith with SSN ending in -1234 would be johns1234)

Default Password - Your date of birth in the following format: MMDDYYYY.You will be asked to change your password after your initial login.

Note: If you are using a shared computer or a computer in a public place, like a library or community center, be sure you close all browser windows and tabs and log out when you're done. This will keep your information secure.
