Username: Your Monument Health Username (Network Login) which may include a number.
Examples:

  • John Smith could be jsmith or jsmith2.

    • Password: If you have never logged in before, it will be the last four digits of your Social Security Number (SSN). You will be prompted to change your password after your initial login. If you have, it will be the one you set up last time you enrolled. Can’t recall? Click “Forgot Password” on the login screen and follow the prompts.


    If you are still unable to log in, contact the Monument Health Benefit Service Center at 844-299-2485. Representatives are available Monday through Friday from 6:00 a.m. to 6:00 p.m. MST.

    HOW THE COBRA PROCESS WORKS
    Your current or former employer offers extended coverage to employees for up to 18 months after termination in accordance with the Consolidated Omnibus Budget Reconciliation Act (COBRA). If you are or were currently enrolled in COBRA eligible coverage (example: Medical, Dental, Vision) as an active employee you may be eligible to continue these coverages through COBRA.
