User name: Date of birth in "mmddyy" format followed by the last four digits of your Social Security Number
Example: Date of birth 06/13/80, Social Security Number 123-45-6789 = 0613806789


Password: Date of birth in "mmddyyyy" format
Example: Date of birth 06/13/1980 = 06131980
