To access the site, please use the following:

Username: first name plus your last name and your date of birth
(Example: John Smith born on 01/01/1990 = johnsmith01011990)
Password: The last four digits of your Social Security Number

Note: If you are using a shared computer or a computer in a public place, like a library or community center, be sure you close all browser windows and tabs and log out when you are done. This will keep your information secure.

Forgot Password?

After repeated unsuccessful attempts to login, you will be locked out of the system. If you have forgotten your password or are having trouble logging in, please click on the "Forgot Password?" link to reset it.
