To access the site, please use the following:

Username: First Initial First Name + Last Name + DOB (MMDD)
Password: The last four digits of your Social Security Number

Example: Tom Smith, 06/14/59 = tsmith0614

You will be asked to change your password after your initial login.

Note: All passwords have been reset as of 10/18/2021. If you are using a shared computer or a computer in a public place, like a library or community center, be sure you log out and close all browser windows and tabs when you are done. This will keep your information secure.
